1. Copy of ID from the Health Care Service Provider from whom you’re working and signed Letter of Employment.
2. Human Resources phone or email address so we can confirm you’re actively working.
3. Address of Home for DPC service.
4. Preferred day, date and time of clean.
5. Client’s email address for service reminder.
6. Client’s cell phone number to contact regarding any changes in booking.
